MGH Cancer Center Health Equity Adv Practice Provider GENERAL SUMMARY/ OVERVIEW The goal of the MGH Cancer Center Health Equity Advanced Practice Provider (HEAPP) is to navigate newly referred patients from local hospitals to receive timely, patient-centered care at the MGH Cancer Center. The HEAPP will help patients of underserved populations, particularly but not limited to those of color, navigate through the MGH Cancer Center and Phase 1 Clinical Trial program. The HEAPP will engage patients and their primary oncologist, create a trusting relationship, navigate patients to MGH Cancer Center appointments, and closely communicate with a patient's entire care team based on clearly outlined goals and identified patient needs. The HEAPP will aid patients in the coordination and completion of appointments inside the MGH Cancer Center, maintain consistent communication with patients and their primary oncologist and help decrease barriers to timely cancer care and treatment. To increase access to high quality cancer care for underserved patients, particularly but not limited to those of color, it will be important to identify and address supportive care needs starting at initial referral to MGH Cancer Center. To achieve this, the HEAPP will be involved in administering supportive care services surveys to all assigned patients, which will help inform the Phase 1 team about the patients' symptom burden, illness perception, financial distress, and self-efficacy and direct future navigation and care coordination efforts. The HEAPP's interventions might include education, coordinating with the Phase 1 Clinic staff to facilitate the following: appointments, interpretive services, help with insurance, transportation, parking expenses, navigating the Cancer Center, assisting in finding appropriate child care, connecting the patient to appropriate clinical trials, and referrals psychosocial and/or palliative care team. The HEAPP will incorporate 1) screening and monitoring for distress, and 2) streamlining referral/early consultation of social work, psychiatry, and palliative care. The HEAPP will be a patient liaison with the MGH oncology team, the patient's referring oncologist and primary care team, ensuring open lines of communication between all teams. The HEAPP will also be involved in other research activities that are related to this program. - Provide competent, compassionate Advanced Practice care based on scientific and nursing principles and the use of nursing process as a primary nursing model - Provide culturally sensitive services to patients from different cultures. Work with medical interpreters to communicate effectively with non-English speakers. - Provide education about cancer clinical trials and when appropriate, assist patient in connecting with care team about available services and resources to support participation. - Work with patients, providers, and MGH Cancer Center care team to set goals for patient's care. - Connect patient with appropriate staff who can address any logistic barriers, scheduling complications, child care needs, etc., that would prevent a patient from completing all of the appointments, tests and scans necessary for complicated clinical trials. - Refer to internal or external case management services when other issues are identified (i.e. hunger issues, domestic violence issues, etc.) - Identify organizational or systematic barriers to care and recommend strategies to improve access - Collaborates with existing MGH diversity initiatives and leadership staff both at MGH and other sites to best assist patients of diverse backgrounds - Identify resources available at MGH, BMC and in the wider community that will assist patients through their cancer care continuum - Address any relevant insurance concerns or issues by connecting patients with appropriate resources and team members. - Assist patient by referrals any necessary supportive care services, psychosocial and palliative care, and social work. - Maintain regular communication with the patient's providers through clinical messages in EPIC and via emails, phone calls and case review meetings. - Provide advocacy, patient education and support in accessing community-based and hospital-based programs. The MGHCC Cancer Health Equity Advanced Practice Provider provides direct care to the clinical trial eligible population in collaboration with the physician and disease center teams. - Performs and records the patient history and physical exam and discusses findings with the physician. - Utilizes advanced practice knowledge to identify, prevent or solve complex and/or recurring patient care problems. - Follows and manages patients (in collaboration with MD) enhancing continuity of care. - Performs (oversees/assures completion of) specialized procedures to the select population mutually agreed upon with collaborating physician. - Performs medically delegated aspects of patient care as agreed upon by nursing and medical administrators per practice guidelines. - Provides first responder coverage for infusion reactions or medical emergencies - Collaborates with unit staff and physicians to facilitate a comprehensive plan of care and coordinate resources. - Writes orders which are in accord with current guidelines. - Maintains and practices in accord with practice guidelines per state and hospital regulations - Provides consultation to both nurses and physicians. - Serves as a resource to staff in area(s) of expertise. - Serves on appropriate intra and interdepartmental committees. - Monitors patient outcomes in accordance with defined standards of patient care. - Collaborates with clinical nurses and medical staff to plan, implement and evaluate pertinent change in practice and health care delivery. - Facilitates patient movement through the system and efficient use of resources through interdepartmental collaboration. - Participates in the improvement of patient care and in the development of formal and informal educational programs for the unit-based nursing and support staff. - Develops educational programs within area of expertise. - Collaborates with unit staff and other health care professionals to meet staff and patient educational needs. - Function as a member of the Clinical Trials team by participating in operational staff meetings and teaching rounds and other meetings as necessary - Interpret hospital and department policies and procedures to health team members - Participate in daily operational activities necessary for safe patient/staff environment - Utilize resources efficiently to facilitate optimal patient care - Provide input to the Program Director and Nursing Director regarding unit needs - Participate in division and department committees for formulation of policies and procedures - Perform monthly QA audit process to measure outcomes of HEAPP interventions with patients - Document every intervention into the appropriate electronic medical record (EPIC, Care Anywhere, etc.) - Produce mid-year and end of the year reports on program activities compiling data from data bases and writing up case examples. - Be involved in evaluation and research activities in the program as needed. - Supports research efforts to advance knowledge and to promote research-based practice. - Evaluates own practice and program effectiveness. - Identifies pertinent nursing and health delivery problems for investigation. - Evaluates current research findings for clinical specialty and determines applicability to unit practice. - Assists nursing and medical colleagues in research efforts. - Encouraged to conduct research - Responsible for own professional growth and maintenance of required credentials. - Maintains the standards and educational requirements specific to the position and licensure. - Identifies own learning needs and goals. - Maintains and updates clinical knowledge and skills based on current health care practice/research. - Contribute to relevant committees, professional meetings, continuing nursing education and/or academic programs - Demonstrated competence in medical oncology - A collaborative and team minded approach with both clinical team and patients. - Ability to navigate resources and partner on best practices. - Ability to effectively communicate clinical information, care plans, etc. to medical and lay audiences; able to educate patients and their families regarding routine and complex medical information. - Skilled in creating appropriate documentation and use of electronic medical records. - Strong organizational and time management skills. - Ability to influence and lead as necessary. Required: - Master's Degree in Nursing (NP) or completion of NCCPA program required (PA) required. - Field of Study/Additional Specialized Training preferred. - 2 Years Advanced Practice experience preferred, prior hematology/oncology experience required - Strong interest in creating an innovative, new role - Passion for working with diverse cultures - Current licensure in the Commonwealth of MA as a registered NP or PA required Preferred: - Experience working with diverse populations - Research nursing experience preferred - Experience conducting outreach in developing strategies to overcome barriers to care preferred, Knowledge of the communities served, the local health system and community resources preferred Massachusetts General Hospital is an Equal Opportunity Employer.
