Holidays and much more! The Director of Quality and Value-Based
Health Care leads Harbor Health Services, Inc. (Harbor) in
implementing quality processes to improve patient care and optimize
organizational performance. The Director of Quality and Value -
Based Health Care continually aligns, develops, and promotes team
based and patient centered care, while ensuring compliance with all
quality reporting to state and federal agencies, and maintaining
recognition programs. The Director maintains relationships with key
business partners and other senior leaders in the industry to
leverage best practices, evaluate emerging technologies, and
distribute knowledge internally to inform plans and strategies.
This role provides oversight and management of operational and
administrative Accountable Care Organization (ACO) activities to
ensure requirements are met in conjunction with the Chief Nursing
Officer (CNO) and Senior Vice President of Community Health Centers
(SVP Of CHCs) and implements strategies to meet financial
value-based quality outcomes and patient/provider satisfaction.
This position will be hybrid (in office and work from home) and is
based out of Harbor Health's Boston locations. Position includes
travel to other Harbor locations as needed. Responsibilities:
Designs, communicates, and implements the annual Quality
Improvement Plan.Works with organizational stakeholders to define
key performance indicators and determines reporting, measurement,
and improvement strategies.Facilitates Quality Improvement meetings
across and within sites to engage care team members in improvement
efforts.Facilitates and/or co-facilitates the Quality Steering
Committee .Uses proven approaches to process improvement to assess
and address systems issues.Analyzes and tracks patterns across
quality data, incident reports and other data collection tools to
identify opportunities for systems improvement.Serves as Harbor's
representative at external forums for quality and BACO (Boston
Accountable Care Organization)Supports state/payor contractual
requirements and reporting including stewardship, quality measures,
and total cost of care, member assignments and provider attribution
(roster). Supports Value Based Contracts and Grants that align with
HHSI's population health, quality, and patient experience
priorities. Collaborates with Clinical Applications Team for the
development of tools, reports, plans and data to support.Ensures
clinical applications tools are aligned with practice standards and
works with clinical leadership team on workflow improvements and
changes as needed.Ensures clinical applications tools support
quality metrics and organizational priorities.Effectively
communicates and manages relationships with other directors -
collaborates on projects, escalates concerns, plans improvements,
manages committees, etc.As appropriate, attends conferences,
workshops to gather best practices and remain current in the
industry.Develops and encourages team-based communication
structures and norming mechanisms to support highest levels of team
performance.Requirements: Bachelor's degree in healthcare related
field, Master's preferred 5+ years of experience in the delivery of
healthcare Proven Electronic Medical Record (EMR)-based analytical
knowledge to detect trends and develop strategic
initiativesExperience working with health care quality improvement
initiatives Knowledge of ACO reporting Proven success in
leading/directing teams to achieve and surpass key performance
metrics including financial quality and provider/patient
satisfaction targetsProven knowledge of Electronic Health Records
(EHRs) OCHIN/EPIC preferredComputer literate with Intermediate
skills in Microsoft Office 365 applicationsFamiliarity with
database structure and querying Proven abilities to manage large
projects and ability to organize and chair meetingsProven abilities
to manage teams of differently skilled individuals in a health care
environmentRespected leadership skills, particularly in the areas
of clinical informatics, quality improvement and partnership with
clinical operationsIn-depth knowledge of EHR applications, user
interface and underlying data structure preferredMust be able to
travel in a timely manner between multiple sites as neededAll
