Job Description:

health. In addition to Community Health Centers, Harbor Health
operates two Elder Service Plan (ESP) Programs of All-Inclusive
Care for the Elderly (PACE), a program that allows frail elders to
live in their homes and communities. Our Elder Service Plan has two
medical and day centers in Mattapan and Brockton, MA and cares for
more than 450 participants. We are seeking a Associate Medical
Director to join our Elder Service Plan leadership team.

Role:
The Associate Medical Director, reporting to the Harbor PACE
Medical Director, supports the medical delivery of the highest
quality clinical care by providers for those patients in the Harbor
PACE program. Additionally, the Associate Medical Director will
maintain a part-time clinical practice while holding a leadership
role within the organization. Additionally, the Associate Medical

Responsibilities:
Leadership
In collaboration with the Medical Director, responsible for
oversight of patient care services, improved health status of
patients, and a safe, culturally sensitive environment of care.
Provides medical guidance and supervision of medical services and
activities, including leadership to the interdisciplinary team by
promoting the goals of the PACE model of care.
Provides leadership and medical expertise in the development of
medical policies, procedures, guidelines and protocols.
Collaborates with Medical Director to achieve and maintain Harbor
Health's benchmarked utilization and cost management (UM) goals and
objectives.
With utilization team, identifies cases requiring elevation to
Medical Director; may assist Medical Director and become the point
of escalation, when appropriate.
Collaborates with Medical Director to ensure optimal provider
performance and productivity.
Participates and supports Quality Improvement Committee (QIC)
initiatives.
Collaborates with the Quality Director to review quality of care
issues and participates in the oversight of the development and
implementation of quality-of-care corrective action plans.
Provides supervision and mentoring of nurse practitioners and new
MDs/DOs, consistent with standards and state regulations.
Conducts competencies for primary care staff.
Develops educational programs to build skill set of provider
team.
Attends and participates in general medical staff meetings and
other assigned committee activities.
Assists Medical Director with special projects, as well as
participates in clinical committees as needed.
Represents PACE to external agencies, professional groups, and
other organizations, as required.
Maintains familiarity with federal and state regulations pertaining
to PACE.
Clinical Care
Provides primary medical management of ESP clients in collaboration
with the physician and other IDT members in the community, ALF,
and/or SNF locations.
Provides assessments of participant's acute conditions (stable
and/or unstable) in the Center, at home, in their ALF or SNF.
Participates in discussions with Interdisciplinary Care Team (IDT)
at daily and weekly care planning meetings and on an as-needed
basis.
Identifies and recommends to the IDT the most appropriate setting
for participant (Home, ALF, SNF, Hospital).
Requirements:
Board-Certified MD/DO in Internal Medicine, Family Practice or
Geriatrics
Current MA MD/DO License, DEA and Mass Controlled Substance
Registration (MCSR)
Current BLS (Basic Life Support for Healthcare Providers)
Certification
Minimum of 3 years of experience in a lead administrative role
Minimum of 1 year of experience working with a frail, elderly or
long-term care population
Health plan and/or health care operations experience preferred
PACE and/or Medicare managed care experience preferred
Utilization management, quality management, performance
improvement, and peer review experience preferred
Passion for working with the frail elderly population required
Must be able to travel between site(s) and client's home in a
timely manner.
