We offer a stable, supportive, and growing organization that focuses on your needs and interests.Well established home care agency has a need for an experienced Home Care RN Case Manager to see patients in homes in a coverage territory to include the Everett/Medford/Malden/Arlington/Belmont area.Full-time, Monday-Friday, 40 hours/weekShorter works weeks consideredNo weekends or on-call required!Competitive pay with mileage reimbursementConsolidated territory, less travelRequires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge.
