We offer a stable, supportive, and growing organization that focuses on your needs and interests.Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager to see patients in homes in the Bedford/Concord/Burlington, MA area.Full-time, 40 hours/week, Monday-FridayOccasional weekend rotation may be requiredNo on callCompetitive pay with mileage reimbursementRequires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge.
