Home Health RN Case Manager OpportunityBeth Israel Lahey Health at Home is seeking a skilled RN Case Manager to join our Home Health team. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.Key Responsibilities:

• Conduct comprehensive assessments of patients and families to determine home care needs.
• Develop and implement individualized care plans that address the unique needs of each patient.
• Administer medications and treatments as prescribed by physicians.
• Supervise and evaluate the care provided by home health aides.
• Identify discharge planning needs and develop plans to ensure a smooth transition for patients.
• Practice confidentiality and adhere to HIPAA guidelines.
• Utilize OASIS and EPIC systems to document patient information and track progress.Requirements:
  • Active Massachusetts RN license.
  • American Health Association BLS certification.
  • Associate's Degree or BSN from an accredited school of nursing.
  • At least 1 year of direct care nursing experience, preferably in public health or home care nursing.What We Offer:
