Provides skilled speech therapy services in an outpatient setting and other appropriate settings consistent with professional standards and policies. Participates in the development of programs necessary to achieve agency objectives as they relate to patient care.

• Completes all documentation required and in accordance with state and federal regulations and policy.
• Perform patient assessments and evaluations to determine functional level and establish a therapeutic program.
• Develop a therapy plan of care with the overall plan of care to include goals and discharge plans.
• Document, within time frames, observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition.
• Communicate and coordinates patient care with other members of the patient's health care team.
• Provide patient caregiver, and family teaching related to the patient's speech therapy program and its maintenance.
• Provide speech therapy in accordance with physician's orders and accepted professional practices.
  
  Qualifications
  
  
  • Masters Degree from an accredited Speech Therapy Program.
  • Licensed in MA.
  • One year of experience in home care, or working with elderly patients preferred
  • Must possess a valid MA driver's license and reliable transportation
  • Demonstrated proficiency with the administration of objective standardized tests and measures and compliance with regulatory standards
