seeking an Outreach HIV Nurse Case Manager who is interested in a
unique opportunity to work on a dynamic, supportive
multidisciplinary team, at the intersections of the HIV,
homelessness, and drug overdose crises. In this role, you will work
with an interdisciplinary HIV team, which includes medical
providers, case managers, social workers, and other HIV nurses to
provide excellent HIV care to homeless individuals receiving HIV
primary care from Boston Health Care for the Homeless Program
(BHCHP). Within this multidisciplinary, patient centered medical
home model, you will conduct clinical nursing assessments and
triage for HIV team patients during routine and urgent medical
visits in the outpatient clinic, as well as during outreach visits
to shelters, drop-in centers, street venues and patients' homes.
You will also provide care coordination, medication adherence
support (including directly observed therapy for HIV mediations),
behavioral risk reduction counseling and harm reduction education
for HIV primary care patients at BHCHP. As the Outreach HIV Nurse
Case Manager, you will have the opportunity to promote harm
reduction, linkage and retention to HIV primary care, with the aim
of supporting people experiencing homelessness and living with HIV
to achieve health and wellness. We are looking to fulfill both
part-time and full-time positions for this
role.Responsibilities:
- Conduct comprehensive intake assessments on new HIV team
patients and along with other nursing personnel, the case manager
and primary care clinician, will formulate a care plan with the
patient; meet periodically with each established patient to assess
their medical and psychosocial needs and update the HIV team with
the patient's care plan; coordinate care with integrated HIV team
behavioral health providers (including psychiatry and
psychotherapy/counseling services) as necessary.
- Track appointments, referrals and primary care activities and
follow up with patients to assure that they keep their appointments
and assist in rescheduling any missed appointments; maintain client
medical records and be proficient in use of BHCHP's electronic
medical record; familiarity with Boston Area substance use disorder
treatment facilities, including detoxification units as well as
residential treatment programs in order to facilitate referrals and
coordinate care.
- Responsible for medication adherence assessment and counseling
and will meet with each patient on HIV medications to ensure they
understand their regimen and will give special instructions as
needed. If indicated, the nurse will supply the patient with pill
boxes and other aids, including directly observed therapy, to
facilitate HIV medication adherence.
- Visit patients at shelters and other outreach sites to provide
nursing care and case management services, as well as to better
understand any barriers to medication adherence and engagement in
care, Collaborate with shelter nurses, transitional facilities
staff, halfway house staff etc. to ensure care is coordinated and
that the patient receives the necessary medication adherence
support.
- Responsible for the clinical quality improvement activities.
Along with the Director of HIV Services, coordinate chart reviews
and other data reviews to ensure that HIV patients are receiving
care according to the BHCHP HIV Clinical Guidelines; assist medical
providers in coordinating activities and follow up related to the
provision of medication assisted treatments (such as buprenorphine
and naltrexone) for opioid use disorder.
- Identify any structural barriers patients face in accessing
medical care and adhering to HIV medications, and will work with
the interdisciplinary team to address those barriers (e.g. housing,
transportation, drug treatment, food, clothing, benefits, etc.);
participate in onsite educational programs as well as participate
in external continuing education programs on HIV related topics on
annual basis, participate in relevant team and program
meetings.Qualifications:
- Bachelor's Degree in Nursing preferred.
- Certification by Association of Nurses in AIDS Care (ANAC) as
AIDS Certified Registered Nurse (ACRN) preferred, however this can
be obtained after hired, and program funding available to support
this training.
- 2 years of experience working in HIV care preferred; expertise
in substance use disorder and harm reduction principles strongly
preferred, experience working with people experiencing homelessness
strongly preferred; Mental Health experience, including knowledge
of trauma informed care, a plus.
- Bilingual in Spanish and English preferred.Compensation and
