ClinicianDEPARTMENT: Behavioral Health ServicesLOCATION: 1575 Blue
Hill Avenue, Mattapan, MA 02126REPORTS TO: Director of Behavioral
HealthTYPE: Full-Time / Part-TimeSALARY: NegotiableJOB SUMMARY:This
position shall have primary responsibility for completing
psychosocial community services under the general direction of the
Behavioral Health Clinical Service Manager in collaboration with
the Director of Behavioral Health Services, in accordance with
policies, procedures and protocols established by Mattapan
Community Health Center. In addition, the incumbent is required to
follow all other appropriate standards of practice, licensing and
certificate, and other regulatory agencies'
requirements.RESPONSIBILITIES1. 90% *Clinical Care*? Perform
appropriate psychosocial assessments and interventions.? Perform
short and long-term therapy.? Create treatment plans and refer
clients to appropriate therapies based on assessment and treatment
plan needs.? Assist in the implementation of community and social
programs as a professional clinician in accordance with Mattapan
Community HealthCenter's grants/contracts with federal, state, and
local agencies and mandates of service requirements with those
grants/contracts.? Support medical providers with getting clients
connected to appropriate behavioral health services.? Perform
crisis assessment and crisis intervention but will review high risk
situations with their clinical supervisor.? Prepare appropriate
transfer and discharge plans and assist clients in a therapeutic
transition from services.? Prepare psychosocial educational
materials for clients, as needed, or required.? Engage with
clinical, health education, community health and other staff
members in the provision of services to patients. Provide referrals
for services needed.? Use tests, measurements, and measurement
scale (PHQ2/PHQ9, GAD7,Vanderbilt Scale, etc.) for general and
specific assessments as well as objective patient criteria in the
provision of services and evaluation procedures.? Will create
patient centered treatment plans, provide ongoing treatment, assess
patient's ongoing clinical needs and progress, and provide
consultation and coordinate care with other designated team
members.2. 10% *Quality Improvement*? Attend bi-weekly Integrated
Team Meetings? Provide patient with external community resources
pertaining to BH services beyond the scope of practice at MCHC.?
Prepare patient reports, correspondence, referrals and other
materials for community agencies, schools, legal proceeding, and
others as needed.? Document telephone contacts via EMR (EPIC).?
Maintain and complete in a timely manner all appropriate clinical
paperwork and documentation in compliance with the Behavioral
Health department policy and procedures, including use of the EHR
and use of DSM 5 coding as needed.? Participate in risk management
meetings, if applicable to case.? Create reports about
patient/client utilization and outcomes, as needed.? Perform other
duties as required by the position or as assigned by the BH

CEO.SUMMARY OF QUALIFICATIONS AND REQUIREMENTS:? Master's in
Clinical Social Work (LCSW or LCSW-candidate, LICSW) or Master's in
Mental Health Counseling (MA or LMHC). Current, valid, and
unrestricted licensure to practice in MA.? Experience in an
ambulatory setting is preferred. Minimum of one to two years of
relevant clinical experience.? Experience working with all age
categories (children, adolescent, and adults).? Ability to be
flexible in working with a wide range of diagnoses.? Successful
candidate must be a self-starter, organized, motivated, comfortable
working in a team-based, collaborative setting and able to respond
to same day visits and/or crisis.? Bilingual in English and
Haitian-Creole or Spanish is a plus.? Good communication skills
with staff and patients.? Excel, outlook and EMR experience
preferred.? Experience in working with diverse culture preferred.?
Familiarity with multicultural groups, previous work experience in
a multicultural setting and willingness to remain current in field
