Community Resource Specialist

Department Description: This is a great opportunity to work as a Community Resource Specialist in the Social Work Department. You will work as part of a multidisciplinary team within Inpatient Medical / Surgical Units and within several ambulatory clinics. Responsibilities include, but are not limited to:

Assessing patient to determine their needs and eligibility for services
Identifying new and existing resources
Providing information and referrals to hospital and community based programs

Developing and documenting a service plan for patients/families
Maintenance of on-line and written resource materials
Consultation to care team about resources and service plans for patients
Assist the team in developing safe discharge plans
