Job Description:
Community Health Network has been placing experienced nurses and
physical therapists in home care, hospice, and case management for
over 30 years. We offer a stable, supportive, and growing
organization that focuses on your needs and interests.We have been
asked to assist one of our top home care & hospice agency partners
with a need for a Hospice Nurse Liaison to be onsite at a major
Boston teaching hospital.Full-time, 36 hours/week3 12-hour shifts
(9:00 a.m. - 9:30 p.m.)Flexible scheduleOccasional weekend may be

include:Assessment of in-patient and determination of eligibility
for HospiceAdmission for Hospice and daily patient visitsCreating
and implementing individualized care plansCoordinating and managing
care with hospital nursing staffServing as Hospice Liaison between
staff, case management, patients and familiesParticipating in
weekly interdisciplinary team meetingsEducating patients and
families about hospicePosition requires:Minimum 2 years Hospice RN
in a Medicare Certified Agency, Inpatient Hospice RN experience a
plus.MA RN LicenseRI RN License (or willingness to apply for it
upon hire)Must be comfortable with issues relative to end-of life
