About the RoleWe are seeking a skilled Registered Nurse to join our team at Mass General Brigham Healthcare at Home. As a key member of our interdisciplinary team, you will provide high-quality, clinical services to patients in various settings within the community.Your primary responsibility will be to deliver patient care that meets the highest standards of community health practice, adhering to our organizational philosophy, policy, and standards. You will be responsible for the admissions process and case management, utilizing thorough and timely electronic documentation and interdisciplinary communications.As a skilled nursing professional, you will provide assessment, teaching, treatment, documentation, and care coordination, demonstrating exceptional customer service and financial awareness. Your ability to work effectively with a diverse population, communicate effectively in writing, verbally, and electronically, and work as a member of an interdisciplinary team will be essential to your success in this role.Requirements

• Graduate of an approved School of Nursing
• Current Massachusetts license as Registered Nurse
• Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred
• Ability to travel within the MGB Healthcare at Home geographic area
• Strong interpersonal and customer service skills, with the ability to work with a diverse population
• Ability to communicate effectively in writing, verbally, and electronically
• Ability to work as a member of an interdisciplinary team of healthcare providers
• Ability to organize and prioritize work, and adapt to changing situations
• Ability to work independently, be self-directed, and adapt to unpredictable circumstances
