Job Description
The RN Case Manager will work with patients, healthcare teams, and insurers to develop and implement discharge plans, ensuring a smooth transition from hospital to post-acute care. The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to work independently.

  The RN Case Manager will be responsible for:

  • Facilitating discharge plans and coordinating post-acute care services
    
  • Developing and implementing discharge plans for patients requiring post-acute care
    
  • Collaborating with healthcare teams, patients, and insurers to ensure a smooth transition from hospital to post-acute care
    
  • Using a collaborative process of assessment, planning, facilitation, and advocacy
    
  • Consulting with physicians regarding medical necessity of admissions and appropriate level of care
    
  • Reviewing patients' health records and communicating with healthcare teams to ensure continued appropriate level of care
    
    The ideal candidate will have:
    
    • 
      
    • Graduate of an accredited school of nursing
      
    • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts
      
    • Minimum of three years' clinical experience
      
    • Experience in healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery
      
    • Knowledge of the special needs and behaviors of infant, child, adolescent, adult, and older adult patients
