Job Description:
This job is for a part-time Hospice RN serving as a Case Manager
within an interdisciplinary team. Responsible for implementing,
coordinating, and delivering hospice services according to agency
policies and regulations. Conducts comprehensive assessments,
educates patients and families, and manages care plans. Coordinates
services, communicates with physicians, and ensures timely delivery
of supplies and medications. Provides support during death and
bereavement, maintains accurate records, and participates in
quality improvement initiatives. Assists in staff development and
program development activities. Requires an RN license, minimum two
years of relevant experience, and BLS certification. Must have
valid driver--------s license, be willing to work in various
community settings, and relocate to Mount Vernon, WA. Offers a
$5,000 sign-on bonus, competitive pay ($40.54 - $73.92/hour), and
benefits including health, dental, vision, retirement, and paid
time off.
