PRIDE Health is seeking a travel nurse RN Home Health for a
travel nursing job in Newton Center, Massachusetts. Job Description

Hospital team is launching a pilot randomized controlled trial to
test the acceptability and feasibility of providing acute
psychiatric care to patients in their homes. Presently, home
hospital programs exclude patients with an active psychiatric
condition; this trial will test the feasibility and acceptability
of Behavioral Health Home Hospital (BHH). The BHH care model will
provide a minimum of twice-a-day in-person nursing to patients,
with remote rounding by the BHH Research Psychiatrist and BHH
Research Psychologist. Additional clinical care team members will
provide care either in person or remotely. Patients admitted and
randomized home as part of the trial will need to have a caregiver
present. Additional support will be able to be provided by home
health aides and mental health specialists. Medication will be
dispensed by the clinical care team, 24/7 monitoring will be
conducted, and patients will be able to join in-patient therapy
groups remotely. The BHH Research Psychiatry Nurse will be a key
member of the BHH clinical care team, as they will provide
in-person hospital/inpatient-level psychological treatment to
patients./n/nJob Summary/nThe Behavioral Health Home Hospital (BHH)
Research Psychiatry Nurse (RN) will support an innovative care
delivery model, Behavioral Health Home Hospital. The program offers
a subset of patients an alternative to standard inpatient
psychiatric hospital admission, in which patients can receive
highly specialized psychiatric-care services in their home. The BHH
Research Psychiatry RN will provide quality, clinical services to
patients in various settings in the community. Patient care is
delivered within organizational philosophy, policy and standards of
nursing and community health practice. The responsibilities are
performed in accordance with Brigham and Womens Faulkner Hospitals
policies and standards of practice, utilizing thorough and timely
electronic documentation and interdisciplinary communications. The
BHH Research Psych RN is responsible for delivering patient care
through the nursing process of assessment, diagnosing, teaching,
planning, implementation, and evaluation of BHH patients. The BHH
Research Psych Nurse can expect to see patients twice a day in
person in their homes or space they are residing in during their
hospitalization. Additionally, the BHH Research Nurse can expect to
have up to 4 patients on service at a time. The BHH Research Psych
Nurse directs and coordinates all nursing care for patients based
on established clinical nursing practice standards and collaborates
with other professional disciplines to ensure effective and
efficient patient care delivery and the achievement of desired
patient outcomes. The BHH Research Psych Nurse will be providing
hospital/inpatient-level treatment to patients in the community.
This is an ideal role for someone interested in a temporary

treatment opportunities for patients with active acute-psychiatric

schedule is established in collaboration with others. Schedules are
routinely adjusted and communicated timely and in collaboration
with the patients home-based clinical care team and is communicated
with the patient and/or families, to meet clinical care and program
needs. /no/tUtilizes multiple data sources to determine
actual/potential health problems and functional status.
