Your Impactful Role as an LPN: As a Licensed Practical Nurse with Loyal Home Care Services, you'll be at the center of personalized care plans - delivering top-tier support and forming trusted relationships with clients and families. Responsibilities: *Deliver skilled nursing care based on the Plan of Care and within scope of MA Nurse Practice Act *Support and carry out care plans initiated by the Registered Nurse *Provide education and guidance to patients and families on condition management and at-home care *Monitor health status, document accurately, and report updates to the clinical team *Collaborate with RNs and coordinate additional services (PT, OT, HHA, MSW, etc.) *Use medical equipment safely, maintaining infection control standards *Stay current on best practices through ongoing clinical development *Be an advocate and source of encouragement for your patients What You'll Need to Join Us: *Active LPN license in Massachusetts *Graduate of an accredited LPN or vocational nursing program * Minimum of 3 years nursing experience (home health or med/surg preferred) * Current CPR/BLS certification *Reliable transportation and valid driver's license *Up-to-date health certificate & TB test *Fluent in English (spoken and written) * Demonstrated knowledge of HIPAA & patient confidentiality *Must be 18 years or older and authorized to work in the U.S. Perks & Benefits * $30 per visit * Health, Dental & Vision Insurance * 401(k) Retirement Plan * Referral Bonus Program * Community Liaison (Patient Referral) Program * Flexible Scheduling & Autonomy in Your Day Now Hiring In: *Franklin County *Worcester County *Hampden County *Middlesex County Ready to make an impact where it matters most?
