Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 02/08/2026 - Duration: 13 weeks - 40 hours per week - Shift: 9 hours, days - Employment Type: Travel ALL APPLICANTS MUST HAVE 1 year Certified Home Care experience. 1 year Case Management Experience. Ability to travel to cases as assigned. Weekend rotations are once every 6 weeks. Job Summary: As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with MGB Home Cares policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
