Job Description:
AccentCare is seeking a dedicated Physical Therapist to join our
team of healthcare professionals in providing exceptional care in
the comfort of our patients’ homes. As a home health physical
therapist, you will play a vital role in improving the mobility and
quality of life of patients who have undergone surgery, experience
chronic pain or illness, or have sustained injuries. This position
requires a blend of expertise and passion, as you will design
tailored therapeutic programs, motivate patients during treatment,
and monitor their progress, all while maintaining our commitment to
compassionate and personalized care. Assess the condition of
patients and develop appropriate treatment plans. Implement
therapeutic interventions for pain management, mobility
enhancement, and overall functional ability. Educate patients and
families on treatments and home exercises. Regularly document
patient progress and adjust treatments as necessary. Collaborate
with other healthcare professionals to provide comprehensive care.
Adhere to all state regulatory and professional standards for home
health care. Strong knowledge of physical therapy principles and
methods applicable to home health care. Excellent communication and
interpersonal skills. Ability to adapt treatment plans to
individual patient needs. Proficiency in using healthcare software
for patient documentation. Medical Insurance Dental Insurance
Vision Insurance 401k Valid Physical Therapist state license. CPR
certified.
