vary), 7:00 AM - 3:00 PM, Day shift Hours per Week: 40 Candidate

Roles & Responsibilities Provide compassionate direct nursing care
to residents, promoting dignity and comfort Administer medications
and treatments accurately and document per facility policy Assess
residents, monitor changes in condition, and communicate findings
to the team Collaborate with nurses, therapists, and support staff
to deliver coordinated care plans Assist with admissions,
transfers, and discharges while maintaining clear documentation
Support rehabilitation goals by encouraging participation in
therapy and daily activities Follow infection control and safety
protocols to protect residents and staff Qualifications Active
Massachusetts LPN license required; active MA RN license accepted
if applying as RN Current BLS/CPR certification required Completion
of an accredited LPN program (or RN program if applicable) Previous
experience in long-term care or rehab settings preferred but not
required Strong communication skills and ability to work within an
interdisciplinary team Reliable, patient-centered, and comfortable
with electronic charting systems Education Graduate of an
accredited LPN program High school diploma or equivalent BLS/CPR
certification Knowledge & Skills Solid medication administration
and documentation practices Basic assessment skills and ability to
recognize changes in condition Familiarity with long-term care and
rehabilitation nursing principles Good infection control practices
and safety awareness Comfortable using electronic health records
and basic clinical tech Strong interpersonal skills and
