Speech Language Pathologist SLP $45/hour admin, $75 routine
therapy, $110 speech assessments | PRN Legacy Lifecare VNA and

reimbursed based on the type of service provided: Office hours
(meetings, in-services, administrative time): $45.00/hr Routine
therapy visits: $75.00 per visit Speech Therapy initial assessment
visits: $110.00 per visit Visit frequency and length may vary.
Schedule: PRN, as needed, flexible scheduling available for evening

Evaluate patients for speech and language disorders, initiate
assessments upon referral, and develop individualized plans of care
in collaboration with the physician and multidisciplinary care
team. Provide rehabilitative speech-language therapy services,
carry out appropriate interventions, and educate patients,
families, and caregivers on treatment procedures and supportive
activities. Monitor, document, and report patient progress,
responses to therapy, and changes in condition using the agency’s
Problem Oriented Record (POR) and required clinical documentation
systems. Coordinate care with healthcare team members, consult with
supervisors, and participate in case conferences, in-services, and
ongoing professional development. Support patients and families in
adapting to physical, mental, and social limitations by providing
education, encouragement, and resources to promote independence and
continuity of care. Maintain compliance with agency policies,
patient rights, confidentiality, infection control, safety
standards, and emergency preparedness while managing workload,
scheduling, and priorities effectively. What You'll Need: Active,
good-standing MA Speech-Language Pathologist license 1-3 years of
experience in geriatrics, long-term care, or short-term rehab
Maintains professional and educational competency in compliance
with state and federal regulations Physically able to perform job
duties, including standing, sitting, walking, bending, squatting,
and lifting/carrying up to 20 lbs unassisted and up to 50 lbs with
assistance Demonstrates strong interpersonal skills, effectively
interacting with residents, families, visitors, government
